Care Management

Morris County provides Care Management to elderly, disabled and frail individuals deemed at-risk of premature institutionalization.

Care managers may link seniors with appropriate community-based services, to allow them to remain home.

We monitor services to address emergent needs, and handle changes that impact the client’s safety and well-being while living in their home.

Services include:

  • personal care
  • home delivered meals
  • transportation
  • respite for caregivers

All services are free of charge (donations accepted).

After a comprehensive assessment is completed, the care managers work collaboratively with the client and/or caregiver to insure and promote choice in provision of service and provider.
