The Morris County Office of Temporary Assistance wants Morris County residents receiving benefits under the Supplemental Nutritional Assistance Program to know that they may be eligible for replacement benefits if their food was destroyed by Hurricane Irene.

Current Morris County SNAP recipients must complete an Affidavit of Loss and submit it to the Office of Temporary Assistance no later than September 22, 2011.

The SNAP recipient must state how and when the food that was purchased using SNAP benefits was destroyed, and estimate its value.

An individual may complete the form at home, print it and bring it with them to the OTA office, 340 West Hanover Avenue in Morristown, or they may fill out the form when they arrive at the OTA office.
